Medicaid Managed Care

• Comprehensive coverage and services for children and adults
• $0 monthly premiums available
• No copay (0% coinsurance) 

Child Health Plus (CHP)

• Comprehensive coverage for children under age 19
• Premiums as low as $0 or $9
• No copays or deductibles

Essential Plan (EP)

• Quality, affordable coverage for adults age 19-65
• $0 premium -- No deductible
• Dental and vision coverage

Qualified Health Plans (QHP)

• Dental and vision coverage options for the entire family
• Telemedicine benefit with 24/7 doctor visits with $0 copay
• HSA-Compatible Bronze option to help cover and lower healthcare costs

HealthierLife (HARP)

• Provide more services for people with serious mental health and substance use disorders. 
• HARPs will also provide all the same services covered by Medicaid Managed Care plans

Fidelis Care at Home (MLTC)

• Home health care, including nursing, physical and occupational therapy
• Private day nursing and personalized care (assistance with daily living tasks)
• Nutritional services, social and environmental supports

Dual Advantage (D-SNP)

• Seamlessly cover health expenses between Medicare and Medicaid 
• No gaps in coverage, one point of contact 
• All for little to no out-of-pocket cost and with monthly premiums as low as $0

Medicare Advantage (MAP)

• Affordable options with as low as $0 monthly premiums
• Innovative benefits, such as wellness and health care planning

Qualified Health Plan Products

Fidelis Care Qualified Health Plan Products are standardized plans offered by Fidelis Care via NY State of Health, the Official Health Plan Marketplace.

Plans fall into various "metal" levels based on their design and the level of cost sharing required: Platinum, Gold, Silver, Bronze, and Catastrophic Coverage. It is also possible to purchase these plans "off-exchange," or directly from Fidelis Care.
