To request a transcript, call our Personal Income Tax Information Center at 518-457-5181, Monday through Friday, 8:30 a.m.–4:30 p.m. Have a tax return at hand so we can verify your identity for your protection. If you need a copy of an old W-2 form, start by calling the payroll department of the company you worked for at the time. If you can’t contact the company, call the person who prepares your taxes and ask them if they can obtain a copy of your old W-2 for you. Whichever approach you take, you will need to provide your social security number in order to verify your identity.
